Special Edition – Register Now-Medical Home/Health Care Transition Trainings – March 22 – April 26
A medical home is a key component to a successful transition to adulthood. These medical home and health care transition trainings scheduled for three locations—Long Beach, Jackson and Tupelo—will benefit parents, advocates, educators and medical professionals. Sponsored by Mississippi Family to Family (MS F2F) and Children and Youth with Special Health Care Needs at the Mississippi State Department of Health, these no cost trainings will help families understand what to expect from a medical home and how this knowledge can help during the transition process.
